This form is used by the McKenzie County Public School District #1 (MCPSD1) to provide a safe and orderly transportation system. All students who require bus service are required to register with Harlow's School Bus Service through this form.

Before completing this form, be sure that you have read and understood the Bus Rider Regulations. You will be asked to verify that you have read and understood these regulations.

PLEASE COMPLETE THIS FORM FOR EACH CHILD WHO REQUIRES TRANSPORTATION.

Students not registered with Harlow's School Bus Service will not be assigned to a bus.

The assigned bus stop may or may not be located at the student's home address. However, students will be assigned within the prescribed walking distance.

Any bus service changes must be made to Harlow's School Bus Service at (701) 444-3626 option 6.
